What information will I need when I schedule my first appointment?

Our appointment secretary will ask you for:

  • the name of your insurance carrier (for example: Blue Cross of Maryland)
  • your policy number
  • the name of the referring physician
  • a brief description of the problem that brings you to our office
  • the name of the place where you’ve already had x-rays or other diagnostic tests (such as a MRI) done for this problem.

If you have a HMO or managed care plan that requires a referral, please contact your primary care physician at least one week before your appointment to make arrangements to obtain your referral. We will need a written referral when you arrive for your first appointment.

What information will I receive before my first appointment?
Our appointment secretary will mail you:

  • a patient medical history form
  • our financial policy
  • our brochure with directions to our offices
  • a HIPAA brochure

What should I do before my first appointment?

  • fill out the patient medical history form
  • if you are having a MRI, complete the MRI screening form.
  • read over our financial policy
  • make a list of all the medications (including the dosage) you are taking. Be sure to include any vitamins or other supplements you take.
  • make a few notes about your symptoms and concerns
  • write down any questions you have

What should I bring to my first appointment?

  • the completed patient medical history form
  • your insurance card and co-pay
  • a written referral form (if required by your insurance carrier)
  • any medical records and diagnostic test results related to your problem
  • the list of your medications
  • your notes and questions
  • any forms which you need us to complete

Current Patients:

Questions & Concerns after an office visit or surgery:
Our patient care staff will listen to your questions and concerns, take them to your physician, and get back to you with his recommendations.

Typical concerns:

  • An increase in pain
  • Above normal temperature
  • A cast that feels tight
  • Redness or warmth around an incision or injury
  • Drainage from a wound or incision
  • Swelling, pain or tenderness, warmth or redness in one or both legs – can a sign of deep vein thrombosis. Call your doctor right away.

Prescription refills:
Some prescriptions can be refilled without an office visit. For routine prescription refills call 301-302-0507 and our staff will obtain the prescription from your physician and call or fax it to the drug store of your choice. We do ask that you allow us 48 hours notice for prescription refills. Narcotics prescriptions can not be refilled by phone.
